In order to provide the highest standard of care, there are times when we may communicate with your other healthcare providers. In signing this form you consent to the collection and dissemination of information as described. You understand that provision of your medical history is necessary to provide you with effective, safe and efficient treatment. You agree to notify our clinic if you have any changes in your health.
Dr Collins’ Fees
Running a private surgical practice safely, efficiently, and reliably requires fees that exceed the Medicare rebates provided by the government. Therefore, there are out-of-pocket expenses for our services. Private health insurance does not cover outpatient costs, however it does help with the expenses of surgery. Our aim is to deliver top-quality healthcare at an affordable cost. A new appointment is $250 ($84.15 medicare rebate). Payment is required on the day of consultation.
